• preovulatory endometrial thickness (day 10-12): > 7 mm
• thyroid function screening: TSH, anti-TG and anti-TPO; if negative antibodies and TSH within normality range (less than 4): no treatment required; if TSH higher than 4 OR positive antibodies: levothyroxine to maintain TSH levels less than 2.5; U/S with normal findings (patients for adenomyosis not eligible)
• other abnormal findings need to be assessed individually
• BMI<33
• non-smoker during treatments
• Requirements for men:
• sperm count: > 15M/mi
• sperm volume: > 1 ml
• progressive motility: > 25%
• normal morphology (Kruger): > 4%
• sperm DNA fragmentation: < 40%
